he'll tell you what's wrong with him". Your history is an integral part of your office visit. Please come a few minutes
early to your initial appointment, and take the time to complete accurately the history form. Be able to tell us at the time
of your appointment what symtoms you are experiences, the duration of the problem, what makes the problem better or worse and
if this is a recurrent problem, how was it evaluated and treated previously.
If you have any suggestions how the doctor or staff of The Grace Family Practice can better serve you, please let us know.
If you have any complaints or concerns, we would appreciate your feedback. We will make every effort to correct the situation.
